VNA of Ohio knows that mental and behavioral health problems are common and influenced by many different factors. 

Mental and behavioral health challenges affect how we think, feel, and act as well as determine how we handle stress, relate to others, and make choices. Our team knows that it is important to pay attention to the signs and symptoms of these challenges so that patients can maintain wellness and decrease unnecessary hospitalizations.

We also know that providing home-based care for individuals is the best method in addressing and eliminating triggers that activate many of these conditions. 

Due to the ongoing pandemic and because hospitals face a reduced capacity to care for these patients, this need will continue to grow. In response, we have innovated and created team- based programs full of resources to assist patients in maintaining autonomy, self-reliance, and safety.

C.A.R.E. Mental and Behavioral Health Program 

A team consisting of a mental health registered nurse, social worker, psychiatric nurse practitioner, Community Health Worker, Behavioral Health Occupational Therapist create a plan to treat patients 18 and older with the diagnosis of a severe mental and/or behavioral health illness and with demonstrated poor coping/maladaptive behaviors/frequent rehospitalization.

  • Assessment of nutritional needs 
  • Establishes safe housing
  • Financial stability evaluation and assistance 
  • Hygiene and clothing assessment and assistance 
  • Individual counseling
  • Medical coverage education, including insurance, physicians, and specialists
  • Medication management and disease process education
  • Providing needed prescriptions
  • Safety plan establishment
  • Transportation, community support, and life skill training
  • Trauma informed care approach

Throughout our service area, Visiting Nurse Association of Ohio accepts Medicare, Medicaid, Veterans Affairs benefits and most private insurances. To best determine your eligibility, please call 1-877-698-6264 to speak directly with an insurance verification representative.
