VNA of Ohio is an independent entity and not for profit organization able to work with patients with and without healthcare, regardless of their material circumstances.

Because we are not beholden to the same requirements as larger healthcare organizations, we have developed a unique program to serve a wide array of patients in need. 

Our holistic NP4U (Nurse Practitioner for You) program unites a patient's primary care physician with our home-based care team of nurse practitioners, visiting physicians, and physician assistants to connect these providers with patients' home life perspectives and material realities. 

NP4U is designed to communicate with patients effectively, along with their caregivers and primary care physicians, through experiences in the intimacy of their own home. VNA of Ohio team members act not only as healthcare coaches to our patients, families or caregivers, but also as central communicators to each patient's existing healthcare team.

Our NP4U Services*

Our team consists of nurse practitioners and primary care physicians who work in tandem with healthcare system physicians to create a plan for each patient's holistic care. 

NP4U service tracks include:

Transitional Care

  • VNA of Ohio contacts patient to assess post-discharge care concerns

  • Creation of comprehensive discharge plan 

  • Ongoing management of at-home based nursing care

  • Communication and transition back to primary physician care


Home-Based Primary Care

  • VNA of Ohio provides primary care services within the home

  • Creation of ongoing health management plan

  • Consultations on at-home care topics such as medicine storage, food shortages, fall hazards, and in home caregiver needs

Palliative Care/Serious Illness Program

  • VNA of Ohio provides serious illness management services in comfort of patient's home 

  • Coaching services provided for care management, palliative care, and advanced care planning.

*VNA of Ohio Home Healthcare is licensed in OH and accredited by CHAP. We are Medicare and Medicaid certified and our skilled services are covered by the patient's health care insurance benefit.

Throughout our service area, Visiting Nurse Association of Ohio accepts Medicare, Medicaid, Veterans Affairs benefits and most private insurances. To best determine your eligibility, please call 1-877-698-6264 to speak directly with an insurance verification representative.
