Throughout our service area, Visiting Nurse Association of Ohio accepts Medicare, Medicaid, Veterans Affairs benefits and most private insurances. To best determine your eligibility, please call 1-877-698-6264 to speak directly with an insurance verification representative.

Visiting Nurse Association of Ohio is certified by the State of Ohio to participate in the Medicare and Medicaid programs. The information below defines and describes key criteria to establish Medicare home care eligibility and should be used as a preliminary guide only.

Homebound

A patient is considered homebound by Medicare if:

  • Leaving the home requires a considerable and taxing effort
    AND
  • Absences from the home for non-medical reasons are infrequent, are for a short period of time, and require the need for assistive devices or the assistance of another person.

Plan of Care

Services must be provided under a Plan of Care established and approved by a physician. The Plan of Care must be updated and reviewed by the physician at least every 60 days, or more frequently if the patient's condition changes.

Skilled Service

The patient must have a need for skilled nursing, physical therapy or speech therapy on an intermittent part-time basis or a need for occupational therapy.

Intermittent/Part-time

To meet the Medicare requirement guideline of “intermittent,” the patient must have a recurring need for skilled nursing, physical therapy or speech therapy at least once every 60 days. Part-time means any number of days per week, up to and including either 28 or 35 hours per week (skilled services and home care aide visits combined) for less than eight hours a day. Intermittent means skilled nursing care that is either provided or needed on fewer than 7 days a week, or less than 8 hours a day for periods of 21 days or less.
