Hospice

Hospice care is for patients who have a life expectancy of months or weeks and wish to have treatment of pain and symptoms (palliative care) rather than curative treatment.

Hospice is a special way of caring for patients at the end of their lives. We believe that our Hospice care should help patients live as comfortably as possible in the end stages of life, and we are dedicated to making every day count for our patients and families. Our focus is not only on the physical needs of patients and their families but also their emotional, social and spiritual needs.

Our Hospice care team includes the best practitioners in the field. That team consists of Hospice-certified nurses, physicians, therapists, social workers, counselors, aides and volunteers who work together with patients, caregivers, families and physicians to provide the highest quality support and comfort. A plan of care is developed for each patient and is updated routinely as the patient’s condition changes.

Our Hospice services include:

• Ongoing medical care oversight and coordination
• Pain and symptom management
• 24-hour availability
• Patient and caregiver education
• Access to a state-of-the art unit for respite and acute care
• Social Work services and referral to community resources
• Hospice-approved transportation
• Specialty hospice services including Alzheimer’s/Dementia
• Coverage for hospice-approved medications, treatments and medical equipment
• Bereavement and spiritual care

VNA Hospice promotes comfort and self-determination, enabling patients to participate in making decisions about care at the end of their lives. Our program is licensed and certified and offers crisis support 24 hours a day, 7 days a week.

We also offer a pre-Hospice program, Special Care Services, for patients with life-threatening illnesses. Patients in this program may continue to receive treatment for their illness.

In addition, The VNA of Ohio Hospice Care Center is a state-of-the-art hospice in-patient facility located at the Senior Health and Wellness Center in Old Brooklyn, Ohio. Designed with solace and relaxation in mind, it provides patients and families with a high-level of care in the comfort of a home-like setting. It has a dedicated team of professionals that includes nurses, physicians, pastoral care providers and specially trained volunteers.

Our in-patient Hospice features:

• 10 private and 2 semi-private rooms decorated in soothing colors, with generous amounts of natural light
• Patient whirlpool and massotherapy
• Overnight amenities for family members
• Art and music studio for patients and loved ones
• Children’s play area
