Palliative Care
What is Palliative Care? It means the treatment of pain and relief of unpleasant symptoms for patients and families living with chronic or life-threatening illnesses or conditions.
At the VNA, Palliative Care is available when you first become ill with a life-threatening disease, when you are receiving treatment, during a chronic disease or at the end of life. We offer the full array of Home Healthcare services, as well as our Special Care Services, which address the unique needs of individuals living with an advanced illness.
Patients and families who can benefit from this type of special care include those with:
- Cancer
- Chronic lung disease or emphysema
- Heart disease or heart failure
- Chronic kidney disease
- Stroke
- Alzheimer’s disease
- Other advanced illnesses such as HIV/AIDS
Our Special Care team of professionals works in collaboration with patients and their physicans in determining patient-centered goals of care with an emphasis on pain and symptom management. Our goal is to relieve suffering, which improves how patients feel on a day-to-day basis and offers the help they need to live as comfortably as possible. In many cases, patients continue to receive treatment for their illnesses, while in other cases we can offer a smooth transition to Hospice.
VNA Special Care Services provide compassionate, skilled nursing and personal care at home. It takes an holistic approach that focuses on the physical, psychological and spiritual needs of patients and familes. In addition, we offer 24-hour crisis support, education and instruction for family caregivers, social work support, and personal care assistance -- all in the comfort of the patient's private residence, nursing home or assisted living center.
