HouseCalls

If you or a loved one has difficulty getting to the doctor’s office because of a chronic, progressive disease, VNA HouseCalls can provide you with the medical care you need – at home. A team of VNA Physicians and Advanced Practice Nurses will bring the doctor’s office to wherever you reside in Northeast Ohio. VNA HouseCalls provides primary medical care and/or consultation with your primary or specialty care physician.

HouseCalls services include:

• Assessment, diagnosis and plan for treatment
• Routine diagnostic treatments and procedures
• Management of pain and distressing symptoms
• Ongoing medical care oversight and coordination
• Patient and caregiver education
• Access to off-hour support and consultation
• Links and referrals to community resources
