Clinical Nurse Specialist/Nurse Practitioner

ADVANCE PRACTICE NURSE
CLINICAL NURSE SPECIALIST OR NURSE PRACTITIONER

IMMEDIATE FULL TIME, PART TIME OR PRN(MON-FRI)OPPORTUNITY

The Advance Practice Nurse provides direct care and services for the HouseCalls patient, including assessment, coordination of services, referrals, etc. The APN maintains an active collaborative relationship with the HouseCalls physicians, and is responsible for the medical management of patients

QUALIFICATIONS

1. Registered Nurse in the State of Ohio; Masters of Science in Nursing and Board Certification in an Advance Practice Nursing Specialty (Adults, Geriatric, Medical/ Surgical, Oncology, Acute Care) by a National Certifying Board (ONCC, ANCC). Current Certificate of Authority to practice in the State of Ohio. Prescriptive authority preferred.

2. Minimum of five (5) years clinical experience in nursing or as an APN. Experience in hospice and home care field preferred. Experience in nursing home/long-term care setting desired.

3. Demonstrated knowledge and expertise in the management of elderly patients with chronic progressive illnesses with an emphasis on pain and symptom control, psycho social management, and disease management.

4. Provide house calls directly to identified patient population, including consultation, care management and monitoring, direct visit assessment, documentation, coordination of services, etc.

ASSISTANT I


House Calls Assistant I
House Calls Office Assistant I is responsible for the coordination of all office and clinical related activities involved in the day to day operations of VNA HouseCalls.

This is an IMMEDIATE Monday - Friday 8am-5pm opportunity

QUALIFICATIONS

1. Associates degree preferred.

2. Two years experience in healthcare office setting required. Knowledge of medical office operations and medical terminology required. Demonstrated knowledge of CPT and ICD coding practices preferred.

3. Ability to analyze and problem solve, ability to manage multiple functions, to maintain efficiency and productivity, and to adapt to change.

4. Must have oral and written communication skills to handle stressed patients and families, and to trouble shoot referral source concerns.

5. Strong computer skills:Word,Word Perfect and internet.

Director Of Nursing
The Director of Nursing plans, implements and directs clinical services and operations to insure the delivery of competent, high quality care and service to hospice and home health patients and families. Provides direction and leadership to clinical management and staff, supports clinical practice, assists with business and program development and monitors and evaluates quality of care and service delivery for hospice and home health programs.

Qualifications:

• Master's degree required preferably in nursing, business or healthcare administration.

HOSPICE BEREAVEMENT COORDINATOR

Hospice Bereavement Coordinator
The Bereavement Coordinator will coordinate services offered to families served by the VNA following the death of the patient. The Coordinator will supervise and coordinate bereavement care services provided by various hospice volunteers, hospice Social Worker staff and will assure adherence to the bereavement plan of care.

IMMEDIATE FULL TIME OPPORTUNITY

QUALIFICATIONS:

Minimum BA degree in social work, counseling, education or related field. MSW, MA, MS preferred.

Training in bereavement work, and experience in providing group and individual counseling or support services.

Strong organizational and communication skills.

Knowledge of community resources including suicide prevention services and appropriate referral mechanisms.

IT - APPLICATION SUPPORT ANALYST

IT - Application Support Analyst
The Application Support Analyst assists in the implementation of, and provides technical support for software applications in use at the VNA. Technical support includes applications, processes and procedures that support the applications and providing technical support for internal and remote users.

Qualifications:
Bachelor’s Degree in Computer Science, Information Technology or Business Management or equivalent experience.

Three (3) years of progressive experience in project management, application and system support, documentation & testing.

Demonstrated experience/knowledge of SQL Server, Oracle, SQL, relational databases, DBA skills, programming tools like Visual Basic, Visual InterDev, C++, ASP, ASP.NET, HTML, XML, Web development and other relevant technologies.

Experience supporting Windows Server 2003, Active Directory, Exchange 2003. Experience with electronic medical records, HRIS and financial & billing software a plus.

Flexibility with work patterns and hours in addition to the ability to execute multiple assignments simultaneously and successfully are essential.

Good communication skills are essential in addition to the ability to collaborate with all levels of personnel within the organization.

Hospice RN Clinical Manager

CLINICAL MANAGER
IMMEDIATE FULL TIME MANAGEMENT OPPORTUNITY

The Clinical Manager, through clinical experience and education preparation has developed experience in community health nursing and/or in Hospice care. The Manager,ensures effective patient care through coordination of staff and appropriate services. The Manager also functions independently in providing comprehensive care to a variety of age and diagnostic groups. Care is directed to individuals and families.

QUALIFICATIONS

1. A minimum of three (3) years experience as a licensed registered nurse, BSN preferred.

2. A minimum of two (2) years experience in community health nursing related to the care of the terminally ill patient.

3. Supervisory experience preferred.

4. Knowledge of and commitment to the Hospice philosophy of care.

RESPONSIBILITIES

1.Provides supervision of clinical staff through coordination of case assignments and other clinical activities by monitoring caseloads, screening referrals, monitoring visit intensity and productivity, assessing skills through joint visits, completing employee evaluations annually, and identifying staffing needs.

2.Provides customer satisfaction through effective communication with clients, families, other health professionals and/or community resources.


3.Ensures adherence to the Hospice Plan of Care in the event of an inpatient admission.

4.Collaborates with staff development to orient new employees, assess training and educational needs of clinical staff and facilitates learning experiences as appropriate.

5.Participates in the QA/UR audits as deemed necessary and ensures that recommendations are implemented.

6.Collaborates with the Director concerning staffing needs and disciplinary actions.

Personal Care Assistant Private Duty

The Personal Care Assistant is an unlicensed para-professional who is supervised by a Registered Nurse in providing personal care, homemaking tasks and companionship for clients. Other HHA duties as directed by the Clinical Manager or designee, include, but are not limited to personal hygiene, cooking, transferring, ROM exercises, catheter care, simple dressings and decubitus care.

The Speech Pathologist is responsible for providing service to homebound patients who have speech and language problems. The direct therapy program is provided under the direction of the attending physician by the Speech Pathologist, with participation from the case manager, family or other caregivers, as necessary.

QUALIFICATIONS

1. M.A. or M.Ed in Speech Pathology.

2. Licensed in the State of Ohio.

3. Certificate of Clinical Competence from the American Speech and Hearing Association.

4. A minimum of two (2) years of recent experience in Speech Therapy, including direct service to patients.

5. Demonstrated competence in speech therapy.

6. The ability to work effectively with a variety of professional disciplines in various settings.

3. Strong organizational skills, ability to interact positively with all disciplines, good communication skills and the ability to function independently.
